Go to Event Forms on your Waffle (you can also add a shortcut tab to your EventSpark menu bar).
Click on New.
Select Custom Survey Form so that EventSpark knows the context of the form and where to place it.
Click Next
Give it a meaningful name.
Select the type of Custom Form, in this case it will be the Survey form.
Complete the fields as well as the Introduction which will appear above the form fields on your new registration form. You can ignore the Search for Delegate fields as this only applies to a Custom Registration Form.

Click Save

You will notice your form has no fields under Fields. To proceed to add fields, click Related.
Step 1:

Click New.
Step 2:
Provide the API Name in the Delegate Field Name field. This is the field name your Salesforce System Admin will need to provide for you to continue adding your desired fields.
Give the field a label, this can be a customized question

If this field is one of your 5-Star Scale fields, select the Input Type 5-Star Scale.
If the field is a Net Promoter Score field, select the Input Type NPS.
If the field you are adding is a text, number, checkbox, picklist or multiselect picklist field, select “Match Field Type” as the Input Type.
Click Save.
Repeat the Step 1 and Step 2 to add any other custom fields you have created.
Remember to ensure your required fields for your survey are on the Delegate/Guest Page Layout in order to access this information later.
